El tumor colorrectal es una de las principales causas de muerte por c\u00e1ncer en el mundo. En los \u00faltimos a\u00f1os, las inmunoterapias, que son tratamientos que reactivan el sistema inmunitario para que ataque las c\u00e9lulas tumorales, han transformado el tratamiento de muchos tipos de c\u00e1ncer. Sin embargo, la mayor\u00eda de los pacientes con c\u00e1ncer colorrectal metast\u00e1sico no responden a estos tratamientos.<\/p>\n

Un estudio del\u00a0Instituto de Investigaci\u00f3n Biom\u00e9dica (IRB))\u00a0y el\u00a0Centro Nacional de An\u00e1lisis Gen\u00f3mico (CNAG), ambos en Barcelona, arroja nueva informaci\u00f3n sobre los mecanismos que limitan la eficacia de estos tratamientos y propone estrategias para mejorarla.<\/p>\n

El trabajo, publicado en Nature Genetics, revela c\u00f3mo, a trav\u00e9s de una hormona llamada TGF-\u03b2, los tumores colorrectales generan una doble barrera que impide que las c\u00e9lulas del sistema inmunitario act\u00faen contra el c\u00e1ncer.<\/p>\n

Por un lado, el TGF-\u03b2 evita que lleguen al tumor\u00a0suficientes linfocitos T (las c\u00e9lulas encargadas de eliminar las c\u00e9lulas cancerosas) desde la sangre. Por otro, bloquea la expansi\u00f3n de las pocas c\u00e9lulas T que consiguen infiltrarse en el tumor.<\/p>\n

La investigaci\u00f3n ha estado liderada por\u00a0Eduard Batlle\u00a0y Alejandro Prados, del IRB Barcelona, junto con\u00a0Holger Heyn, del CNAG,<\/p>\n

\u201cGracias a la secuenciaci\u00f3n de c\u00e9lulas individuales dentro del microambiente tumoral, hemos identificado a los principales actores afectados por el TGF-\u03b2\u201d, se\u00f1ala Heyn. \u201cCon tecnolog\u00edas de vanguardia, observamos c\u00f3mo esta mol\u00e9cula bloquea la eficacia de la inmunoterapia e identificamos nuevas dianas terap\u00e9uticas para optimizar los tratamientos contra el c\u00e1ncer colorrectal\u201d.<\/p>\n

Dos muros que bloquean las defensas<\/h2>\n

El estudio combina modelos experimentales de met\u00e1stasis en rat\u00f3n con an\u00e1lisis de tumores humanos. El objetivo era entender c\u00f3mo el TGF-\u03b2 articula la resistencia a la inmunoterapia, un fen\u00f3meno que ya hab\u00edan observado previamente.<\/p>\n

Los investigadores descubrieron que el TGF-\u03b2 act\u00faa como una se\u00f1al de \u201cprohibido el paso\u201d: impide que las c\u00e9lulas T circulantes, capaces de atacar el tumor, accedan a la met\u00e1stasis. Al mismo tiempo, modifica los macr\u00f3fagos para que produzcan osteopontina, una prote\u00edna que frena la proliferaci\u00f3n de las pocas c\u00e9lulas T que logran entrar. Esta doble acci\u00f3n convierte al tumor en pr\u00e1cticamente invisible para el sistema inmunitario.<\/p>\n

\u201cEn nuestros modelos experimentales, cuando bloqueamos el TGF-\u03b2, las c\u00e9lulas inmunitarias pudieron infiltrarse masivamente en el tumor y recuperar su capacidad de ataque\u201d, explica Ana Henriques, primera autora del trabajo. \u201cAdem\u00e1s, al combinar este bloqueo con inmunoterapia, observamos respuestas antitumorales muy potentes\u201d, a\u00f1ade Maria Salvany, tambi\u00e9n coprimera autora.<\/p>\n

Nuevas l\u00edneas terap\u00e9uticas<\/h2>\n

Aunque existen\u00a0ensayos cl\u00ednicos con inhibidores de TGF-\u03b2, su uso en pacientes sigue limitado por los efectos secundarios. Este estudio sugiere estrategias alternativas, como bloquear los mecanismos activados por el TGF-\u03b2 \u2014entre ellos la producci\u00f3n de osteopontina\u2014 para lograr un efecto similar. \u201cEn cualquier caso, estas aproximaciones deber\u00e1n evaluarse en ensayos cl\u00ednicos y siempre en combinaci\u00f3n con inmunoterapia\u201d, comenta Eduard Batlle.<\/p>\n
